症例８　10歳代　男性：投球時の肩の痛み

類上皮血管腫 epithelioid hemangioma

 

Epithelioid hemangioma

・血管腫瘍の1つ（類上皮血管内皮腫や血管肉腫を含む）で非常にまれ。
　30-60代に好発し男女差なし。
・軟部組織に発生、骨は2番目の頻度
・長幹骨の骨幹端もしくは骨幹発生

＊良性腫瘍であるが以下の特徴がある！
　─18-25％は1つの骨に多中心性発生する
　─リンパ組織に浸潤する報告あり（→ 骨巨細胞腫と類似した傾向）
　─10%は再発する（これで死亡例なし）

画像の特徴
・単純X線写真・CT：骨硬化縁をもつ境界明瞭な骨透亮像
・MRI
　　T2WI：不均一ながらも高信号
　　T1WI：筋肉より若干高信号
＊均一な増強効果
＊浮腫・骨皮質の破綻はあっても良い

 

まとめ

・Epithelioid hemangiomaの1例を報告
・若年者・骨端発生もあり得る
・単純X線写真で骨硬化縁がみえづらい場合はrapid growthの可能性が高い
・腱の付着部近傍に腫瘍が存在すると運動時の痛みを起こすので注意
