| 髄外造血（骨髄脂肪腫） extramedullary hematopoiesis（myelolipoma） |
| 鑑別診断 |
脂肪肉腫
・脂肪成分が主体で鑑別にあがる
・軟部濃度は脱分化巣の可能性あり
・脱分化巣としてはFDG集積に乏しい点は非典型か
髄外造血
・脂肪成分が主体で非典型な所見
・背景に明らかな血液疾患を疑う所見に乏しく、順位は下がる
| 髄外造血 |
・正常な造血幹細胞が骨髄以外の部位で増殖
・多くは背景に血液疾患を伴う
・肝臓や脾臓に好発
・胸腔内発生は稀だが、胸腔内では後縦隔、特に傍脊椎領域の報告が多い
・無症状であれば経過観察可能

・成熟脂肪細胞と正常骨髄系細胞からなる良性腫瘍
・主に副腎皮質に発生
・15％が副腎外発生
副腎外のうち約半数が仙骨前に発生、縦隔は8％程との報告あり
・合併症がなければ経過観察可能

・正常成人では脊椎や胸骨、骨盤、頭蓋骨、肩甲骨、大腿骨および上腕骨近位部に集積
・髄外造血、骨髄脂肪腫ともに集積あり

| Take Home Message |
・脂肪を含む後縦隔腫瘍の診断として髄外造血/骨髄脂肪腫を念頭に置くことが重要である
・術前診断にはIn-111骨髄シンチグラフィが有用である可能性がある
・術前に診断できれば、不必要な手術や処置を回避し、経過観察することも可能かもしれない
